By The LeanGLP Team · Label information verified 2026-08-05
Wegovy is semaglutide, a once-weekly injection; once-daily 25 mg oral tablet also on the 2026 label from Novo Nordisk. Chronic weight management, cardiovascular risk reduction in adults with established cardiovascular disease and obesity/overweight, and noncirrhotic MASH — per the prescribing information (June 2026 revision).
Dose-escalation schedule (per the prescribing information)
| Period | Dose | Label note |
|---|---|---|
| Weeks 1-4 | 0.25 mg once weekly (injection) | |
| Weeks 5-8 | 0.5 mg once weekly | |
| Weeks 9-12 | 1 mg once weekly | |
| Weeks 13-16 | 1.7 mg once weekly | |
| Week 17 onward | Maintenance dose (see below) | Label: if a dose isn't tolerated during escalation, consider delaying escalation for 4 weeks. |
Maintenance dosing
Weight reduction in adults: 1.7 mg or 2.4 mg (recommended) once weekly. For adults who tolerate 2.4 mg for at least 4 weeks, when additional weight reduction is clinically indicated, the label allows increasing to a maximum of 7.2 mg once weekly (Wegovy HD). Tablets: 1.5 mg daily (days 1-30) → 4 mg (31-60) → 9 mg (61-90) → 25 mg maintenance from day 91.
Plain-language notes
- The June 2026 label covers both the weekly injection and a once-daily 25 mg oral tablet with its own 30-day escalation steps (1.5 → 4 → 9 → 25 mg).
- Tablets are taken on an empty stomach with up to 4 oz of water, at least 30 minutes before eating or drinking anything else — a real scheduling constraint the injection doesn't have.
- Pediatric patients 12+ (injection): maintenance 2.4 mg (recommended) or 1.7 mg.
Why the slow ramp exists
Every GLP-1 label escalates gradually for the same stated reason: reducing gastrointestinal side effects while the body adapts. For nutrition planning, the escalation window is when appetite suppression tends to hit hardest — which is exactly when protein intake usually craters. That's the problem our GLP-1 protein calculator and week-one meal plan are built for.
