Creatine is having a moment in GLP-1 circles, and for once the internet's enthusiasm points at a supplement with an actual evidence base. Here's what that base supports, what it doesn't, and the caveats that matter for this specific population.
Position up front: if you're doing resistance training while losing weight on a GLP-1, creatine monohydrate is a reasonable, cheap, well-studied addition — after protein and training are handled, and after a quick conversation with your prescriber. If you're not lifting, it isn't the thing to buy.
What the evidence actually supports
Creatine monohydrate is arguably the most-studied sports supplement in existence. - The scale will blip. Creatine pulls water into muscle cells — during loading, body-mass increases of roughly 1-2 kg are reported in the literature, and it's water, not fat. On a weight-loss medication this is purely cosmetic, but if you don't know it's coming, it can wreck a week's motivation. Skip loading if that bothers you.
- Kidney context matters. Creatine supplementation modestly raises serum creatinine, which can muddy kidney-function labs, and people with existing kidney disease should not add it without clinician sign-off. Healthy-adult safety is well documented in the position stand; "healthy" is the operative word.
