Orforglipron — brand name Foundayo, approved April 2026 — is the first oral small-molecule GLP-1 receptor agonist, and it arrived with a lot of "does everything change now?" energy. For nutrition specifically, the honest answer is refreshingly boring: almost nothing changes, and that's the point.
What it is
Foundayo (Eli Lilly) is a once-daily tablet for chronic weight management in adults with obesity, or overweight with a weight-related condition. Unlike the peptide GLP-1s (semaglutide, tirzepatide) that require injection or — for oral semaglutide — strict empty-stomach dosing, orforglipron is a small molecule that survives digestion, so it's taken with or without food, any time of day, swallowed whole. What it changes for nutrition: convenience, not chemistry
The oral form removes two real frictions, both about scheduling rather than what to eat:
- No injection logistics. Nothing to store cold-chain, no shot day. Doesn't touch protein needs.

The trial doses you'll see cited (6, 12, 36 mg) don't match the tablet strengths on the label (0.8 through 17.2 mg) because they describe different chemical forms — the approved tablets are labeled by orforglipron free base, and the label notes the 17.2 mg tablet corresponds to the 36 mg calcium-salt dose used in trials. It's a labeling convention, not two different medications, and it has zero nutrition implications — but it explains the confusing dose mismatch if you go reading the trial paper.
